3. Clinical Forms and Payment Submission

Comprehensive Preparation for a Precise Diagnosis

You will receive your secure invoice, which is due before the date of the appointment, read more on our flexible payments and other assistance. Simultaneously, you will complete the necessary digital forms, which are critical for the clinical work:

• Short Intake Form: A focused questionnaire (the interview will expand on this—no long intake forms).
• External Reports: Rating Scales to be completed by self and, importantly, a parent/spouse/significant other to gather comprehensive historical and current observations for differential diagnosis.
• Consent Forms: Standard consent for evaluation and telehealth services.
• Release of Information (ROI): Forms needed to use and share your results with your doctor, school, or other providers.

4. Virtual or In-Person Session: Interview, Testing, & Feedback in one day

Objective, Comprehensive Evaluation, Done Right in One Day

Log onto your computer or laptop for the full virtual session or arrive on time at our office. This single appointment includes the diagnostic interview, the secure Core-Flexible cognitive and psychometric testing (including objective measures of attention, exective functionig, memory, visual spacial, academic achievement), and the final verbal feedback session with immediate recommendations. All complex clinical work is completed efficiently in one day.

5. Receive Your Final Report and Action Plan

Actionable Documentation, Same Day for Treatment and Accommodations

Your comprehensive, detailed, and actionable report is delivered to you electronically by the end of the business day following your virtual session. This document explicitly includes all treatment recommendations, accommodation guidance (for IEP/504 or ADA), and necessary referral support, giving you the immediate next steps you need.

6. Superbill for Out-of-Network Reimbursement

Support for Your Insurance Claim

After the evaluation, we will provide you with a Superbill—a detailed, itemized receipt containing all necessary diagnostic and service codes (CPT codes) for professional services. While we do not interface directly with insurance, you can submit this Superbill to your carrier to seek out-of-network reimbursement, significantly reducing your final out-of-pocket cost. You can use Thrizer for superbill support with a small fee.
